When anatomy and bone allow, the implant is placed in the same sitting as the extraction. A provisional crown may be fitted the same day — but only if the implant's primary stability, measured intra-operatively, clears the protocol threshold.

What it means

Two concepts.
Often confused.

Immediate placement

The implant is positioned in the same sitting as the extraction, using the post-extraction socket. Preserves bone volume and removes one surgical visit from the schedule.

Immediate loading

A provisional crown is fitted the same day. Possible only if the implant's primary stability, measured intra-operatively with an ISQ device, exceeds the protocol threshold.

Selection criteria. Not every case is a candidate. The post-extraction socket must be intact, with no active infection. Residual bone must allow sufficient primary stability. Occlusion must permit the temporary to be unloaded. The decision is taken intra-operatively — if the criteria are not met on the day, we revert to the delayed protocol. We do not promise immediate loading from a distance.

Indicated

Intact socket · adequate bone

  • ✓ Residual bone volume verified on CBCT
  • ✓ Occlusion compatible with unloaded provisional
  • ✓ Primary stability above ISQ threshold (intra-op)
Not indicated

Active infection, thin bone, severe bruxism

  • · Unresolved peri-apical infection
  • · Insufficient residual bone
  • · Severe bruxism without occlusal guard
  • · Heavy parafunctional load

If the criteria are not met, we use the delayed protocol: extraction, two to three months of socket healing, then conventional implant placement. The clinical outcome is equivalent.

Diagnosis & CBCT

Same sitting. ISQ reading taken intra-operatively. Provisional only if stability is sufficient.

Final restoration

After three to six months of osseointegration: definitive E-Max or zirconia crown.

Written warranty

10 years on the implant fixture.
5 years on the ceramic crown.

Return flight covered for documented complications attributable to the clinical work, written into the treatment agreement and not a marketing phrase. Choosing the immediate or the delayed protocol does not change the warranty terms.
