Ozempic Teeth: The UK Dental Guide to GLP-1 Medications and Your Mouth

More than 500,000 UK patients are now prescribed a GLP-1 medication — Ozempic, Wegovy, Mounjaro or Saxenda — either through the NHS weight-management pathway or through private prescription. For most of them, the conversation with their prescriber does not include dentistry. It should. The cluster of oral-health changes now known as Ozempic teeth is real, it is under-recognised in the UK, and it affects every dental decision you will make for as long as you are on the medication. This guide explains what happens, why, and what changes in your dental plan.

What is “Ozempic teeth”?

“Ozempic teeth” is a lay term used since 2024 to describe a constellation of oral-health problems reported by patients on GLP-1 receptor agonist medications. The most commonly reported symptoms are:

  • Persistent dry mouth (xerostomia) — the most consistent finding, reported by 30–40% of long-term users.
  • Rapid onset tooth decay — multiple new cavities appearing in patients who previously had stable oral health.
  • Enamel erosion — thinning of enamel, particularly on the inside surfaces of upper front teeth, from repeated acid exposure.
  • Increased tooth sensitivity — cold and sweet sensitivity, often the first warning sign.
  • Gum inflammation — early gum disease, sometimes without the usual bleeding on brushing.
  • Halitosis — persistent bad breath resistant to normal hygiene.
  • Tooth fracture — in severe cases, fractures of previously restored teeth.

The term is not in official medical literature. The phenomenon, however, is being observed widely enough that the American Dental Association, the British Society of Dental Hygiene and Therapy, and individual clinicians worldwide are publishing practice advisories.

Why do GLP-1 medications affect teeth?

The mechanisms are understood, even where the causal chain is not yet proven in clinical trials:

1. Reduced saliva production

GLP-1 agonists slow gastric emptying to produce satiety. The same autonomic pathway appears to reduce parasympathetic stimulation of the salivary glands. The result is chronic dry mouth. Saliva is your primary defence against tooth decay — it buffers acid, delivers calcium and phosphate to remineralise enamel, and contains antimicrobial peptides. When saliva drops by 30–50%, decay risk multiplies.

2. Acid exposure from nausea

Approximately 16–20% of GLP-1 users experience nausea; 5–9% experience regular vomiting, particularly in the first three months. Each episode of vomiting coats the inside surfaces of the upper front teeth with stomach acid (pH 1–2). This is the same erosion pattern seen in patients with bulimia or chronic GORD. Enamel is demineralised at pH 5.5; at pH 2 the erosion is rapid.

3. Reduced acid reflux symptom awareness

Even where overt vomiting is not present, silent acid reflux is increased in GLP-1 users because of delayed gastric emptying. Many patients do not experience heartburn, so they do not know the reflux is happening. The teeth, however, do know.

4. Nutritional deficiencies

Rapid weight loss reduces intake of calcium, vitamin D, vitamin C and phosphorus. All four are essential for tooth and periodontal health. Deficiencies weaken enamel and predispose to gum disease. The combination of dry mouth plus nutritional deficiency is particularly aggressive.

5. Behavioural changes around eating

Patients on GLP-1 medications often graze on small carbohydrate snacks to manage nausea (crackers, toast, small portions of fruit). Frequent carbohydrate exposure without saliva to clear it is the exact recipe for rapid caries. Patients who previously ate three meals a day and had stable oral health are now eating six small portions and developing cavities between every tooth.

What should you tell your dentist?

This is the single most useful action you can take. If you are on any GLP-1 medication, tell your dentist at the start of every appointment. Specifically tell them:

  • Which medication (Ozempic, Mounjaro, Wegovy, Saxenda, Rybelsus) and the dose.
  • How long you have been on it.
  • Whether you are experiencing dry mouth, nausea or vomiting.
  • Any change in taste, sensitivity, or new tooth pain since starting the medication.

This information changes the plan. Specifically it should change:

  • Recall frequency — from 6-monthly to 3-monthly, because decay progresses faster.
  • Fluoride protocol — from over-the-counter toothpaste to prescription-strength (5,000 ppm) fluoride, sometimes with a fluoride varnish applied quarterly.
  • Saliva-stimulating protocol — sugar-free gum, saliva substitutes (Oralieve, Biotene), increased water intake.
  • Dietary counselling — specifically the frequency of carbohydrate snacks, and the acid-exposure pattern of drinks (sparkling water is mildly acidic; fruit teas are strongly acidic).
  • Restoration material choice — bonded composites and porcelain veneers both rely on healthy enamel to bond to; in a GLP-1 patient with acid erosion, bond strength may be compromised.

GLP-1 and veneers: what changes

This is the specific question we are asked most often at GC Clinic. The short answer: veneers can still be done, but the plan changes.

  • Pre-treatment stabilisation. Before any veneers, we want at least three months of controlled oral environment — fluoride protocol in place, saliva restored where possible, acid exposure minimised, any active caries treated.
  • Enamel assessment. An experienced clinician can identify the characteristic erosion pattern of GLP-1-related acid exposure. Where erosion is significant, a veneer bonded to partially demineralised enamel has a shorter lifespan than normal. We may recommend composite bonding first (cheaper, easier to refresh) rather than porcelain.
  • Minimal-prep strategy. Removing less enamel is always the preference; in a GLP-1 patient it is essential. A full-prep Hollywood-style veneer is contraindicated in most GLP-1 cases — there is not enough enamel to work with. A minimal-prep old money smile is usually the right approach.
  • Extended warranty reasoning. The 5-year written warranty that we give on crown and veneer work is predicated on stable oral biology. For GLP-1 patients, we record the medication in your file so that if the warranty is called on, there is clinical context. We still honour the warranty — we just want the record to be accurate.

GLP-1 and dental implants: what changes

Implants are affected differently from veneers. The bone is not directly damaged by GLP-1 medication. However:

  • Osseointegration timing. Some data suggest slightly slower osseointegration in rapid weight-loss patients, likely due to nutritional deficiency rather than the drug itself. We extend the loading protocol by 4–6 weeks for patients who have lost more than 15% of body weight in the preceding year.
  • Peri-implant hygiene. Dry mouth increases the risk of peri-implantitis (gum inflammation around implants). Patients on long-term GLP-1 therapy need a more aggressive maintenance protocol — typically a 3-monthly specialist hygienist appointment.
  • Protein intake pre-surgery. Bone healing requires adequate dietary protein. We ask GLP-1 patients to confirm with their prescriber that protein intake is sufficient, specifically in the fortnight before and the six weeks after implant placement.

Is it safe to stop GLP-1 medication before dental work?

Do not stop your medication without discussing with your prescriber. GLP-1 drugs are usually prescribed for medically significant reasons — diabetes, obesity, cardiovascular risk. Discontinuation has its own risks, including rebound weight gain and glycaemic instability. The dental plan adapts to the medication; the medication does not adapt to the dental plan.

If a surgeon (for significant implant surgery) specifically requests a short pause, it should be coordinated with the prescribing physician — not decided by the patient alone.

Practical protocol, if you are on GLP-1

  1. Tell your dentist. Today if possible.
  2. Brush with a 5,000 ppm fluoride toothpaste (Duraphat 5000 is the UK NHS-available option; Colgate Prevident 5000 equivalent privately).
  3. Do not brush immediately after vomiting — rinse with water, then wait 30 minutes. Brushing demineralised enamel accelerates loss.
  4. Sip water throughout the day; avoid frequent sipping of acidic drinks (sparkling water, fruit tea, squash).
  5. Use sugar-free chewing gum (xylitol-containing) to stimulate saliva.
  6. Book 3-monthly hygienist visits rather than 6-monthly.
  7. If planning cosmetic dentistry (veneers, bonding, whitening), stabilise the oral environment first. Plan on a 3-month lead-in.
  8. If planning implant surgery, confirm protein intake with your prescriber.

Why dental tourism patients need to raise this specifically

In a 20-minute consultation in a high-volume clinic, your GLP-1 prescription is unlikely to be asked about. In a specialist-led boutique practice, it is part of the first-contact medical history. At GC Clinic, the intake form includes an explicit GLP-1 question since Q4 2024. If you are considering treatment abroad, make sure the clinic knows; the ones that take it seriously will change the plan accordingly. The ones that do not take it seriously are not the ones to trust with your teeth.
