All-on-4 in Turkey: What Actually Happens, Day by Day

The All-on-4 procedure replaces an entire arch of teeth with four implants supporting a fixed bridge. For UK patients flying to Istanbul it is a two-visit treatment over six months — and most of the difference between a successful case and a complication is in the planning, not the surgery itself.

Who All-on-4 is for, and who it is not

All-on-4 was developed for patients who have lost most or all of the teeth in one arch and either cannot wear a denture comfortably or do not want to. The four-implant geometry — two straight at the front, two angled at 30–45° at the back — distributes load across the available bone without grafting in roughly 80% of cases.

It is not a universal solution. If you have severe bone loss in the upper jaw and the maxillary sinus has dropped, you may need a sinus lift or a zygomatic implant alternative. If you have unrestored gum disease, that has to be treated first. And if your remaining teeth are restorable, removing them to fit All-on-4 is overtreatment — we do not extract healthy teeth to fit a treatment plan.

The CBCT comes first, always

A 3D CBCT scan is the foundation of any All-on-4 plan. It shows bone height, bone density, the position of the inferior dental nerve in the lower jaw, and the maxillary sinus floor in the upper jaw. Without it, implant angles are guesswork. We will not place implants without a current CBCT — yours, taken in the past 12 months — even if the case looks straightforward on a panoramic X-ray.

For UK patients, a CBCT scan is available at most private dental surgeries (£150–£250) and at imaging centres in major cities. We accept the digital file directly via WhatsApp or upload before you book a consultation, so the planning starts before you fly.

Visit one: the surgical day

Visit one is typically eight days in Istanbul. The structure looks like this:

  • Days 1–2: Arrival, in-person consultation, planning verification, pre-surgical hygiene, blood work if indicated.
  • Day 3: Surgery. Extractions if needed, implant placement under local anaesthesia (IV sedation available on request), fixed acrylic temporary bridge attached the same day.
  • Days 4–7: Recovery in your hotel, daily check-in via WhatsApp with photographs, two in-clinic reviews on day 5 and day 7.
  • Day 8: Final post-op review, departure home with the temporary bridge in place.

The surgery itself takes 90–180 minutes per arch. Both arches in a single appointment is possible but more demanding on the patient — we typically split bilateral cases across two surgical days for comfort.

Visit two: the final bridge, six months later

Osseointegration — the biological fusion of implant to bone — takes approximately six months. The temporary bridge protects function and aesthetics during this period. At six months, the implants are stable enough to support the final, full-strength zirconia bridge.

Visit two is four days in Istanbul: scan, try-in, fit, occlusion check, polish, photographs, departure. The final zirconia bridge is milled in our partner laboratory using the same CAD/CAM workflow used for crown work — there is no compromise in material because it is the second visit.

Materials: what we use and why it matters

For All-on-4 cases at GC Clinic, the standard implant tier uses Neodent or Medentika (both Straumann Group). The premium tier uses Straumann BLT or Nobel Active. The VIP tier uses Straumann BLX with guided surgery and PRP for accelerated healing. Lot certification is provided for every implant placed — you receive the document at discharge and a copy is logged in your records pack.

We do not use unbranded or “Korean” implant lines on All-on-4 cases. The reason is not snobbery: the four-implant geometry depends on long-term bone-implant interface stability, and the published 10-year data exists for Straumann, Nobel, Neodent and a handful of others. For a case where four implants must succeed for the bridge to function, we use systems with documented long-term outcomes.

What complications look like, and how we handle them

Implant failure rates for All-on-4 in published Straumann/Nobel data run at 1–3% per implant over 10 years. In practice, that means the typical patient does not experience failure, but the case must be designed assuming one might fail. Our protocol: if a single implant loses osseointegration in the first six months, we replace it before the final bridge is fitted. This is covered by warranty and includes return-flight costs.

Other complications — peri-implantitis (inflammation around the implant), prosthetic screw loosening, occlusal wear on the temporary bridge — are managed with scheduled follow-up. Six-month, 12-month and 24-month checks are part of the standard package.

UK aftercare: the records pack and your local dentist

Before you leave Istanbul, you receive a complete records pack as a PDF: implant brand and lot numbers, surgical site map, bone graft material if used, prosthetic specifications, occlusal scheme, and a treatment timeline. This is what your UK dentist or hygienist needs to maintain the work. Frequently asked questions

How many days do I need in Istanbul for All-on-4?

Visit one is eight days for a single arch. Both arches in one trip is possible but we typically suggest splitting the surgical days for patient comfort. Visit two is four days, six months later. The total time off work is therefore about two weeks across two trips, not one long stay.

What if I do not have enough bone?

About 80% of All-on-4 candidates have enough bone for the four-implant configuration without grafting — the technique was specifically designed to use the bone that remains after tooth loss. For the 20% who need additional bone, options include a sinus lift (upper jaw), bone graft (lower jaw), or zygomatic implants for severe upper-jaw atrophy. Which implant brand will be used?

Standard tier: Neodent or Medentika (Straumann Group). Premium tier: Straumann BLT or Nobel Active. VIP tier: Straumann BLX with guided surgery. The brand is confirmed in your written treatment plan before you book. You receive lot certification for every implant placed at discharge.

Will my UK dentist take care of my All-on-4 work?

Yes for routine hygiene, gum maintenance and emergencies. For specific work on the implants or bridge, we do that. Most UK private and NHS dentists are comfortable with maintenance work on All-on-4 cases when they have the records pack — that is what the pack is designed to provide.

How much does All-on-4 cost in Turkey vs the UK?
